Once on the website, you will see a highlighted icon that says "Updated PPE Form." That form is the Preparticipation Evaluation Form that is required to be obtained by all students to participate in UIL atheletic activites and that includes marching bands. This includes the preparticipation and medical history all in one form. It is a state requirement that all students that participate in band and colorguard need to complete every 2 years.
